DHA Season Registration 2022/2023

Congratulations on successfully earning your spot at the Delta Hockey Academy! Now that you have been selected for one of our teams, you will need to complete the 4 step online registration process. This registration should only take about 10 minutes to complete.

Please note: this registration page is for individuals that have officially been offered a spot on a Delta Hockey Academy team and have signed a Letter of Intent. If you have not been offered a spot on a roster and are interested in learning more about our academy, please visit our Prospective Student-Athletes page.  

Step 1: Delta Hockey Academy Online Registration

Player Information
This information will be used for team travel bookings.
Please complete this section in the following format: NUMERICAL MONTH/DAY/YEAR. For example, December 31, 1990 should be submitted as 12/31/1990.
For players, please indicate which hand you shoot. For goalies, please indicate which hand you catch with.
This information will be used for the CSSHL online roster. Please use the following format for the CSSHL: if you are 5 foot 1 inches tall, please type 5.01 & if you are 5 foot 10 inches, please put 5.10.

I understand that it is my responsibility to keep the team Safety Person advised of any change in the above information as soon as possible. In the event of a medical emergency and that no one can be contacted, team management will arrange to take my child to the hospital or a physician if deemed necessary. I hereby authorize the physician and nursing staff to undertake examination, investigation and necessary treatment of my child. I also authorize release of information to appropriate people (coach, physician) as deemed necessary.
Delta Health & Performance Clinic
The information in this section will be shared with the Delta Health & Performance Clinic management. For those who are new to DHA, the Delta Wild Hockey Group manages the Delta Health & Performance Clinic located at Planet Ice Delta. It's an integrated sports medicine clinic with a team of healthcare practitioners dedicated to integrating the highest standards of physiotherapy, chiropractic therapy, athletic therapy, massage therapy and mental performance training. Should your son or daughter need additional care for an injury that occurs on the ice, you will be able to utilize healthcare coverage provided by Hockey Canada up $500 per player in a given season for physiotherapy and athletic therapy. Please note that if your child already has coverage through your own extended benefits plan, Hockey Canada requires that the family utilize that coverage first before the $500 will kick in. If your child is not covered for physiotherapy or athletic therapy under your family plan, then you can opt in to using the $500 coverage from Hockey Canada. Our medical staff will fill out the necessary Hockey Canada paperwork so that any expenses incurred by your family for physiotherapy or athletic therapy that is required in the recovery process will be reimbursed to you by Hockey Canada. We hope this support mechanism will help families cover the cost of any injuries.

DHA Tuition Payment Information
Tuition instalments are collected over 7 monthly payments due on the first day of each month (September 1, 2022 – March 1, 2023). See our Program Fees page for detailed tuition payment schedules for each team (deltahockeyacademy.com/register/program-fees/). Please indicate your preferred payment method for monthly tuition instalments below:
I agree to fulfill the total program fee for my child’s assigned team according to the payment schedule. Payment will be made to the Delta Wild Hockey Group by post-dated cheques, online payment, or direct deposit. If an invoice is not paid when due, this could result in suspension of the student-athlete from ice time. If payments are outstanding for more than seven days, the family will be subjected to a flat rate administration processing late fee of $50. The Delta Hockey Academy reserves the right to treat unfulfilled payments as a breach of this agreement. If a player withdraws from DHA to play for a Junior A or WHL team, he will not be obligated to fulfil the remainder of his monthly tuition instalments.

All new and returning DHA athletes must complete the form linked below for the Canadian Sport School Hockey League (CSSHL). You do not need to print a copy of the form. It will be submitted electronically to the CSSHL once you hit “submit” at the bottom of the form.

Step 4: North Delta Secondary School Registration

For all new DHA student-athletes, please click on the link below to complete your academic registration with North Delta Secondary School. If you are a returning DHA student-athlete, you do not need to complete this step.
